Camp Registration

By signing this youth camp consent form agreement, I understand and agree that my child will be participating in various activities, both indoor and outdoor, that may involve physical exertion and potential risks. I acknowledge that the camp staff and organizers will take reasonable precautions to ensure the safety and well-being of all participants, including my child, but I understand that accidents can happen. I authorize the camp staff to provide emergency medical treatment for my child in the event of an accident or illness. I also give permission for my child to be photographed and/or videotaped during the camp activities for promotional purposes. As a parent or legal guardian, I agree to indemnify and hold harmless the camp staff, organizers, and sponsors from any and all claims, damages, or expenses arising from my child's participation in the camp activities.
